  • We have partnered with FinalForms, an online forms and data management service that allows families to begin the registration process online. If you require any support during the process, scroll to the page bottom and click “Use Support”.

    Only parents/guardians are permitted to register their child. In cases of divorce or separation, only the custodial parent/guardian can sign paperwork for your child. 

    Getting Started With Final Forms

    IMPORTANT: After completing the online registration process on FinalForms you will also need the following materials:

    • Birth Certificate

    • Immunization Record

    • Driver’s License

    • Proof of Residency: Most recent electric or utility bill - must have your name, address and date on it. (If you are living with a relative or friend, bring their most recent electric or other utility bill - must have their name, address and date on it.)

    • In the case of divorce/separation: Legal document showing proof of custody

  • To register your kindergarten student, please call during the following dates and during the hours listed below each school:

    Jefferson Elementary:
    3/16/26 – 3/27/26 between 10am – 3pm
    or
    4/6/26 – 4/10/26 between 10am – 3pm

    Rock Creek Elementary:
    3/16/26 – 3/27/26 between 11:30am – 3pm
    or
    4/6/26 – 4/10/26 between 11:30am – 3pm

    Kindergarten children must be 5 years of age on or before the first day of school.

    Only parents/legal guardians are permitted to register their child.  In cases of divorce or separation, only the custodial parent/legal guardian can register the child. 

    IMPORTANT: During this call, please be prepared to give your child’s full name, mailing & street address, date of birth, and parent/legal guardian’s home/cell/work phone numbers.

    All registration is to be completed through Final Forms. Please ensure you select the 2026-2027 school year when registering. 

    Copies of the following documents will need to be dropped off or mailed to the school to complete registration:

    • Birth Certificate
    • Immunization Record - State law requires a complete series of the following: 
      DTAP, IPV, MMR, Hepatitis B, Chicken Pox  (Complete Immunization Record is required at the time of registration.)
    • Proof of Residency 
      Bring your most recent electric or other utility bill - must have your name, address and date on it. (If you are living with a relative or friend, bring their most recent electric or other utility bill - must have their name, address and date on it, in addition to a piece of mail with the parent/guardian’s name on it.)
    • Legal document showing PROOF OF CUSTODY if a divorce or separation is involved.
    • Custodial parent/guardian driver’s license

    Developmental screening:

    An appointment will be set up for a date in May when a series of tests will be given to your child. The areas screened will be Language Arts and Math. The screening program will not exclude any child from school that is of legal age; however, it will help parents make informed decisions about their child’s school career. It is important to register your child during these dates/times so that we can plan classes for the fall. Thank you.
